Please bring to the appointment:

• Primary and secondary insurance cards
• A photo identification card
• The completed Influenza Vaccine Clinic Registration form

• Aetna Whole Health - Virtua
• Ambetter from WellCare of NJ (commercial exchange plan)
• AmeriHealth Commercial, AmeriHealth Local Value Network, AmeriHealth Hospital Advantage EPO
• Empire BlueCross BlueShield Commercial - unless it is a PPO plan, which is indicated on the card with a suitcare symbol
• NaphCare
